Students who participate in Lake Country Christian School sports programs must complete a Sports physical and sign the required forms from the athletic office. All forms must be turned into the Athletic Office. Click on the forms below to print them.

All of the following 4 forms need to be printed and completed for High School Sports. Middle School students must fill out the Physical, Medical History and the Team Commitment form. They do not need to fill out the TAPPS Student Acknowledgement of Rules Form.
